Street Lane Practice is happy for new patients to register with the practice if they live within our catchment area (see below). If you are already a patient and are moving locally please contact reception for advice. You may still be eligible to stay with the practice.
Start the registration process online. Registration with the practice usually takes up to 48 hours to be put on the system. After this you will be able to book appointments. Each family member will need to register with the practice individually. If you need any assistance please ring reception.

Please note that once your registration is complete, it may take 3 to 5 business days for your repeat prescriptions to be added to our system.

You do not need to know your NHS number to use NHS services, but it can be useful to have it.
The practice boundary is:
- North - Wigton Lane
- East - Roundhay Park Road, Princes Avenue, Roundhay Road
- West - Harrogate Road, Moortown, King Lane onto Gledhow Valley Road
- South - Gledhow and Roundhay meet
